1. Obtain referral forms from the link below, have pages 1 to 6 filled out by a family member, referring agent such as a counsellor, mental health and addictions worker, probation officer, or doctor.
2. Have medical portion of the referral form, pages 10 to 15, filled out by your family doctor or walk-in clinic doctor.
3. Fax the forms to Miklat office fax line at 604-483-9397 or alternatively email them to [email protected]
4.We will call you to confirm an intake date when your forms are received in our office and we have approved you for treatment.
